Without a classroom assessment is difficult to ascertain what might be happening. The red dots on the neck and shoulder could be caused by a host of ailments, including psoriasis, impetigo, zoster (shingles), an arthropod bites (eg ticks) or nummular eczema. And, without a face to face assessment and unable to perform certain tests can not determine whether the points and shoulder pain are related to his previous elbow surgery. If your symptoms have not yet been resolved and if you have not gone to the doctor, I recommend you do so soon.
